Colorado Family Medicine Residency PCMH Project
Colorado Clinical Guidelines Collaborative (CCGC), the University of Colorado Department of Family Medicine, the Colorado Association of Family Medicine Residencies (CAFMR) and the Colorado Institute of Family Medicine (CIFM) are partnering together to begin working on the challenging task of practice transformation in Colorado’s family medicine residency programs.
The “Colorado Family Medicine Residency PCMH Project” originally grew out of the experience CCGC has had with working with multiple types of practices throughout the state in practice redesign efforts, including a couple of residency practices. Early experience led to the idea that assisting a residency practice in developing characteristics of sustainable improvement work would require a joining of forces between a residency’s practice and program, with tandem efforts supportive of the improvement work within the curriculum itself.
The PCMH is an approach to providing continuous, comprehensive, coordinated care with a partnership between patients and their personal healthcare team, as a part of an integrated medical neighborhood.

- NCQA PPC-PCMH Recognition Program™ application fee
- Onsite practice improvement coaching
- Curricular redesign assistance
